I'm swapping a 3sgte from a rhd st205 to a lhd st205. My mechanic says that the wiring in my car has been tampered with, and is useless. Is it possible to use the entire ecu-wiring harness from the rhd if i just extend the cables a bit?

Yes, you can extend the wiring and use it. Stick with the JDM ecu too cause you get a few more HP out of it because of the tune on the ECU

Can i simply replace my stock ECU with the jdm ECU? Without any further adjustments?

You should be able to since it's for the same engine.
Not sure if someone else can chime in on this?
